Provided with resources including: social services, housing assistance, crisis hotlines, urgent care, mobility/transportation information, utility assistance, sanitation, illness prevention, and skills for socialization.

Health Home

About

Provided to individuals with chronic conditions that are unable to leave their home easily to access health services.

Offers care coordination services including psychiatric rehabilitation, mobile treatment and opioid treatment services.

Helps empower you to manage and prevent chronic conditions and improve health outcomes while avoiding preventable hospital encounters.

Residential Housing

About

5-bed residential group home that provides therapeutic services and 24-hour staffing for females with behavioral, emotional, and mental health difficulties.
Youth are required to attend school, participate in structured recreation programs, attend individual and group therapy and engage in pro-social behaviors.

Accepts referrals from Baltimore City and all surrounding Maryland Counties Department of Social Services.

Eligibility:

Adolescent females ages 15-19.

About

Provide community-based assistance to improve quality of life. We assist and empower you to develop your life skills and work towards recovery.

Suboxone Maintenance

About

Suboxone contains a combination of buprenorphine and naloxone, used to treat opiate addiction and dependence. Suboxone along with a comprehensive behavioral health treatment allows better adherence to using opiates.

Medication Management

About

Involves physicians and patients working to ensure that drugs are being used as prescribed while educating about what the drug is doing and mental illness.

We use treatment such as: performing patient assessments, making medical treatment plans, ensuring compliance through patient education, and communicating between medical professionals and patients to provide high quality care.

Outpatient Mental Health Care

About

Provides comprehensive behavioral health services in the home, school, and community.

Diagnostic assessment for: Mental Health and Substance abuse conditions, Individual therapy, Family therapy, Group Therapy, Case management, & Referral for Psychological evaluation.

Medication management and collaboration with other medical providers is used as needed.
