Transitional Housing (Vivienda de transición) (Level 3.1)

About

Transitional housing. Length of Stay is 6 months -1 year.

Fees:

Yes -- Medicaid and out of network insurance is accepted. (Tarifas: Si -- Se aceptan Medicaid y seguros fuera de la red.)

Audience:

Women at least 21 years of age in recovery from drugs and/or alcohol. (Personas Atendidas: Mujeres de al menos 21 años de edad)

Eligibility:

1) Must be able to pay program fees 2) Have a minimum of 30 days clean or completed a treatment program of six months or more in recovery (Emergency situations will be addressed on a case by case basis 3) Provide a copy of your discharge summary and continued care plan 4) Make available all doctors evaluations, medical and psychiatric reports 5) Submit for review all prescribed medications 6) Have a sponsor or get one in the program 7) Have a physical examination (exams from your treatment center will be accepted) 8) Follow all house regulations and participate in random drug analysis.

How to Apply:

Women are admitted to our program through a referral process and are approved by an assessment. To receive housing: 1) Refer client using the referral form 2) Provide Biopsychosocial and discharge information 3) Provide any and all medical information 4) Send all requested documentation via email to (intake@lightoftruthcenter.org) or fax to (866-910-5074) 5) Schedule a telephone intake and a tour of the facilities 6) After the intake and tour is complete, clients will be notified within 48 hrs. of acceptance/non-acceptance 7) Move in date, location, and instructions will be given upon acceptance. About

Before gaining employment you will complete the Aftercare Program to establish a foundation of recovery and provide support for the first six months.
