General Information


Residential services for men and women, addiction and mental health services, group and individual counseling, transportation and substance abuse treatment.

We offer Telehealth services, have Spanish speakers in the center, and healthcare related trainings such as CPR and CMT training.

Supported, safe and supervised housing services. Individuals in housing include: those with select chronic medical conditions, ongoing mental health needs, and developmental challenges.

Supportive Employment Program (SEP)

About

Providing a support system, skills, and links to the community. Program Includes: Assessment and Needs Identification; Community Partnerships; Skill Acquisition; Job Placement; Ongoing Support.

Primary Care and Weight Management Services

About

Provide primary care and medically supervised weight management services to focus on patient needs and preferences.

This consists of: Health Promotion including Immunization; Annual and Other Physicals; Diagnostic Testing; Treatment and Management of Acute and Chronic Conditions; Rehabilitation; Palliative Care.

Options available include: Nutritional Counseling and Supplementation; Weight Coaching and Meal Planning; Phentermine; B12, Lipotropic, Glutathione, and HCG Injections; AOD TrocheIt.

About

Psychiatric Rehabilitation Program (PRP)

About

Offers individualized care to people with mental issues. We help you to improve daily living skills necessary to support recovery.

Some areas of focus in the program include: Scholastic advocacy and support; Educational skill building; Vocational readiness and support; Financial literacy; Social skills and support; Assertive communication strategies.

Outpatient Mental Health Center (OMHC) (Mental Health Services)

About

Outpatient Mental Health Center (OMHC) provides mental health services and individualized care for patients of all ages.

Includes: Diagnostic Evaluations; Psychiatric Evaluations; Individual,; Family, Couples/Relational, and Group Counseling/Therapy; Medication Management; Mobile Treatment Services; and Court-Ordered Care.

DUI/DWI Education

About

A (6) two-hour session for individuals who are encouraged from the court system to complete a DWI/DUI (AEP). Upon completion, you are given a certificate of completion for the 12-hour Alcohol Education Program.

Medication Assisted Treatment (Withdrawal Management)

About

Provides opioid and alcohol maintenance treatment with Suboxone, Subutex, Zubsolv Vivitrol, Sublocade-injectable buprenorphine, and Other Medication Assisted Treatment.

Intensive Outpatient Program (IOP) (Level 2.1)

About

A 26-week treatment at our office that helps individuals with substance abuse education, prevention, intervention, relapse maintenance, emotional management, and recovery.

Outpatient Services (OP) (Level 1)

About

26-week treatment that incorporates both group and individual sessions 2 times a week (90-120 min) at our office. Helps with substance abuse education, prevention, intervention, relapse maintenance, emotional management, and recovery management.

Partial Hospitalization Program (PHP) (Level 2.5)

About

Treatment program for those experiencing behavioral or emotional problems due to substance abuse. Consists of 30 hours of treatment per week (or five days per week, approximately six hours per day).

For clients who are not in need of 24-hour inpatient service, but are at high risk for relapse or require control and support for a drug-free lifestyle.

Residential Treatment (Level 3.5)

About

Offer chore services, companion services, personal care services, and respite services.

Care is available twenty-four hours a day and seven days a week.
