General Information

The Community Asthma Program (CAP) helps Baltimore City kids ages 1 to 18 who have a hard time managing asthma. Trained health workers visit your home to help with:

* Learning about asthma medicine
* Using medicine the right way
* Getting an Asthma Action Plan from your doctor
* Finding asthma triggers in your home
* Giving supplies to help lower asthma triggers
* Connecting you to other helpful services
