General Information


Opioid outpatient treatment program that provides guidance and support for housing, employment assistance, and volunteering opportunities to help with recovery from substance abuse and mental health issues. Distributes Narcan and provides free trainings.

Services


Medicated Assisted Treatment (MAT)

About

Reduces the cravings and physical symptoms of opioid withdrawal with Methadone, Suboxone (Buprenorphine), or Vivitrol. These medications block the effect of opioids to treat substance use disorders.

Audience:

Adults - 18 years old and older.

Eligibility:

Must demonstrate active alcohol and drug use that can be supported by blood, oral swab or urinalysis testing.
Medical Services

About

Create a plan of care with a provider based on your physical and medical needs and wants. Plan must adapt to what you need throughout treatment.

Mental Health Wellness

About

Psychiatric staff, social workers, and counselors offer group treatment, one-on-one treatment, and medication to support your health. You will learn new skills for managing addiction and mental health problems.

Psychiatric Rehabilitation Program (PRP)

About

A rehabilitation treatment program that provides the skills and support to improve wellness, quality of life, and purpose.

Program aims to improve: relationship stability; coping skills; academic & employment skills; time management; emotional well being; and family & community integration.

Audience:

PRP services offered to minors (children & adolescents) and adults.
Outpatient Program (OP)

About

Outpatient Program is the most independent level of treatment offered at CCG. Helps overcome marijuana addiction, alcohol addiction, gambling, sex addiction, or other areas of need.

Intensive Outpatient Program (IOP)

About

If you have been receiving outpatient treatment and need to increase the level of support, IOP can be helpful. Includes individual and group counseling and clinical programming with topics 2-3 hours long offered four times per week. Focuses on handling alcohol and drug cravings, communication skills, employment, health knowledge, and relationship building.

Can be virtual or in person.

Partial Hospitalization Program (PHP)

About

Offers a more intensive level of care for times when extra support is needed. Allows you to live at home while getting the same level of daily support as an inpatient or residential treatment program.

Receive individual, group, and family therapy, educational classes, and skill building activities.

The length of your treatment program depends on: your specific needs as determined by your initial assessment, your clinical treatment team, and the progress you make in your recovery.

Population Health Management

About

An important part of combining mental health and substance use treatment, providing information and resources you need to manage your health. The treatment team of Population Health Nurse Care Managers focuses on diet and exercise, social supports, income, job skills, and meaningful activity.

Behavioral Health Therapy

About

Behavioral Health team treats opiate addiction by: engaging the patient in their drug abuse treatment, changing attitudes and behaviors, creating positive habits to resist triggers, and developing life skills for stress. Treatment is customized to the individual based on their life conditions and needs.

Audience:

Individual and family therapy for adults and adolescents. Group therapy and psychiatric services are available for adults only.
