HealthCare Access Maryland can help you get health care coverage and assist you with healthcare needs. They can help you find doctors, find transportation to medical appointments, and even food resources. All navigation and care coordination is being conducted remotely.

About
Community health workers can help you understand your health benefits, find providers in your health insurance network, and answer any questions you may have about your health insurance coverage. They can also help you find free or low-cost transportation options to get to and from medical appointments. If you need food, they can help you find long term food resources. Call the center and ask to be connected with a communtiy health worker

About
All care coordination is remote.
Navigators are workers who can assist you in applying for health insurance or renewing your health insurance. They can help you enroll in, renew, or make changes to your Medicaid, and also answer any questions you have about your coverage. To speak with a Navigator, please call (410) 500-4710 and listen to the prompts. If you need to complete your call in Spanish, click 1. If you need help with Medicaid, click 4. If you need help with private health insurance plans, click 5.
Please be aware that Mondays and days after a holiday are their busiest days, so your wait time might be longer.

If you are experiencing a crisis, Mental Health Specialists and Addiction Counselors at this center will listen carefully to your concerns and connect you with resources that can help you. By calling the hotline at 410-433-5175, you can speak to a crisis worker and get the assistance you need. You do not need to be insured.

Maternal Assistance
About
This program offers social support to women are pregnant or have given birth. An outreach worker come to your home and provide you with health education and resources so that you can better care for your baby. They can even deliver a new portable crib to your home and show you how to put it together, explain how to keep your baby safe when sleeping, and provide other tips to ensure the health of your baby. If you are interested in any of these services, call 410-649-0500 and click option 2. Foster Care: Making All the Children Healthy
(MATCH program)About
A committed team of registered nurses and licensed social workers and healthcare professionals who: * coordinate medical exams * coordinate dental exams * coordinate behavioral health exams * enroll children/youth in MD medical assistance insurance program (MA) * case-manage children’s complex behavioral and physical health needs * support health needs of pregnant and/or parenting youth * assist young people in assuming responsibility of managing their own health care when transitioning from foster care into independent adult lives.

Community Diabetes Initiative
(CDI)About
The Community Diabetes Initiative (CDI) is a program in Baltimore that helps people lower their risk of getting type 2 diabetes. It is run by HealthCare Access Maryland and works with the Baltimore City Health Department.
Services of CDI include: * Education: They teach people about prediabetes and how to prevent diabetes. * Screening: They offer tests to see if someone is at risk for diabetes. * Support: They connect people to programs that help them make healthy changes, like eating better and being more active. * Resources: They help people find services for healthy food, transportation, and other needs.
Diabetes Prevention Program (DPP) The DPP is a 12-month program that helps people make healthy changes to prevent diabetes. It includes: * Healthy Eating: Learning to eat better. * Physical Activity: Being active for at least 150 minutes each week. * Weight Loss: Losing at least 5% of body weight.
Eligibility:
To participate in the DPP, you must: * Be 18 years or older * Have a Body Mass Index (BMI) of 25 or higher (23 o4 higher if Asian American) * Not be previously diagnosed with type 1 or type 2 diabetes * Have received a high-risk result (score of 5 or higher) on the Prediabetes Risk Test * Not be pregnant
