Health Home
About
This program helps people in opioid treatment take care of their overall health. Staff help you make doctor appointments, learn about your health, and manage long-term illnesses.
You can get help with regular check-ups, special health needs, and mental health care. Some basic medical care, like treatment for hepatitis C, is also available.
The goal is to make it easier for you to get the care you need to stay healthy.
